Reablement care, also known as intermediate care, is a short-term home care service designed to help individuals regain their independence after a hospital stay, accident, or illness. Unlike post-operative or post-surgical care, reablement care is specifically aimed at people who will recover their independence within a defined period of care, usually lasting no longer than six weeks.
During this time, an integrated care team comprising of professionals from all areas of the NHS and the social care team will work together to provide support in the home. This may include mobility support, help with daily tasks, and medical care provided by homecare workers, nurses, occupational therapists, physiotherapists, social workers, and doctors. By focusing on increasing independence, reablement care empowers individuals to take control of their recovery and achieve their goals.
